Located in the heart of downtown Madison, Maplecrest Rehabilitation and Living Center offers a full range of health services from post-acute transitional care between hospital and home, to long-term nursing care. We provide outpatient therapy to the community as well as our inpatient services. Adult day and hospice services are also available. Our programs are individualized, physician-directed, and goal-oriented, enabling our patients to achieve their highest levels of independence.

Small in size, Maplecrest Rehabilitation and Living Center has a convenient in-town location and a one-story floor plan. Our bedrooms, dining rooms and lounges are tastefully appointed and impeccably maintained. Beautiful landscaping and patios have earned Maplecrest North Country's Award for Outstanding Facility Appearance.

Meals are nutritious and appealing. Our menus are carefully developed by a registered dietitian, food service director and the patients themselves.

Our stimulating, well-rounded activities program ranges from current events discussion groups and exercise classes to barbecues, music and films. Outings in our wheelchair-accessible van for lunch, shopping, day trips, and other special events are always a highlight.

Weekly religious services provide an opportunity for patients to fulfill their spiritual needs.

DIRECTIONS:   

From Farmington:
Take Route 43 East or Route 148 East into Madison. Maplecrest is on the left, 0.3 miles from the street light.

From Skowhegan: Take Route 201 North to Route 148. Turn left onto 148 and proceed approximately 6 miles. Maplecrest is on the right, across from Main Street Elementary School.

From Interstate 95: Take Exit 133 (Old Exit 36) onto Route 201 North to Skowhegan, then Route 201A through Norridgewock into Madison. Turn right at the light onto Main Street. Maplecrest is 0.3 miles on the left, across from Main Street Elementary School.
